Here is Bread (here is wine)

This is a gentle hymn about Jesus present in the Eucharist

It was written by English evangelical worship leader and contemporary folk-rock singer and songwriter, Graham Kendrick (b 1950) for the album Spark to a Flame in 1991, and first published by Make Way Music.

It is set to an unnamed tune, also by Kendrick. This has a 4/4 time signature. In the key of C, the first few notes of the melody are EG G_  CE E_  EF E D  EF E D   with accompanying chords  C /  G  / Am / E7  / F  /   C  /  G  / F   / Am / G.

Lyrics

The lyrics are copyright so cannot be reproduced here. But they are currently available on the author's website. A rough outline is:
Here is bread, here is wine
Christ is with us, he is with us:
Break the bread, drink the wine
Christ is with us here.

Here is grace, here is peace
Christ is with us, he is with us:
Know his grace, find his peace
Feast on Jesus here.

In this bread there is healing
In this cup there’s life forever:
In this moment, by the Spirit
Christ is with us here.

Here we are, joined in one
Christ is with us, he is with us:
We’ll proclaim, till he comes
Jesus crucified.
